FMCSA requires a minimum of $750,000 for non-hazmat freight under 10,001 lbs GVWR. For hazmat, the minimum jumps to $1M–$5M depending on commodity. Factors that affect cost include: your driving record, years of authority history, vehicle value, cargo type, annual mileage, and state. Fleet programs for 2+ units typically offer lower per-unit rates. New ventures (less than 2 years of authority) pay a significant surcharge that phases out after 24 months of clean operation.

Yes — most carriers will write physical damage on trailers regardless of age, though older trailers (15+ years) may be limited to stated value coverage rather than ACV.
